Number of Employees Base Rate:

1 to 4 $150 5 to 10 $200 11 to 20 $250 21 to 50 $350 51 to 100 $500 101 to 200 $1,000 Over 200 $1,000 + $1 for each employee over 200 (Maximum $2,500)

Schedule B

Hotels, Motels, Convention Centers, Resorts, Apartments $150 + $2 per unit

Schedule C

Real Estate and Insurance $150 + $30 per agent

Schedule D:

Professionals (Doctors, Dentists, Lawyers, Physicians, Assistants and Nurse Practitioners $150 per professional employee in firm

Schedule E-1:

Non-Profit Organizations* (non-business related), Educators, Nurses, Ministers, Individuals $75 Note: individuals cannot be employed; non-profit should be 501C3 or 501C6

Schedule E-2:

Senior Citizens $75

Schedule F:

Financial Institutions $15 per million dollars in assets ($1,500 Maximum)

Schedule G:

Utilities Negotiable ($1,500 maximum)

Each member firm, association, corporation, partnership, or estate shall be entitled to a number of such designees as determined by the amount of their annual paid as outlined below:
